Recommendations

Operators of small mines should review their safety management systems and the findings of risk controls assessed in the Regulators inspection program:

Criteria assessment Recommendations to industry
Are there inspections in place at the Quarry to support the safety management system (SMS)?
  • Ensure inspections are scheduled, undertaken and provide reports on the effectiveness of risk controls in site safety management systems
Who completes the inspections and are they trained to complete the inspection?
  • Ensure all persons undertaking site inspections are trained and competent to inspect and report on the effectiveness of risk controls in site safety management systems
Who reviews completed inspections?
  • Ensure a system of review of completed inspection reports is undertaken by competent persons and all non-compliant issues are assessed and rectified within an appropriate time
How often are inspections completed? Is it appropriate for the size and complexity of the operation?
  • Ensure the site inspection system is fit for purpose to suit the mining, crushing, processing and transporting operations undertaken at the site
Is there a rectification process for issues that are found on inspections? Are workers empowered to stop the job till identified issues are rectified?
  • Ensure a system of review of completed inspection reports takes place and monitored in a defect management system to rectify identified non-compliance
  • Ensure workers and supervisors are trained and competent in recognising non-compliant site operations and have assigned authorisation to shut down site operations as required
