The Quarterly safety report – July to September 2025 has been released to inform industry when reviewing safety management systems and undertaking risk assessments at their sites. The report details incidents and trends relating to hazard management, in addition to sector specific information.
As a high-hazard regulator, we focus on compliance with legislative requirements associated with principal and other high-risk hazards. ‘Roads or other vehicle operating areas’, ‘fire or explosion’ and ‘air quality or dusts and other airborne contaminants’ are hazards that feature regularly in incident notifications to the Regulator.
In this reporting quarter, there were a total of 596 incident notifications received — an 18% increase from the previous quarter and a 1% increase from the equivalent period one year earlier. There was one non-work-related fatality during the quarter.
Incident notifications increased for coal mines (+10%), extractives mines (+38%) and metalliferous mines (+57%) compared to the previous quarter. The only sector to decrease was opal mines which dropped from one incident to zero.
Assessments increased by 27% overall this quarter from 927 to 1175.
Safety notices decreased slightly this quarter from 710 to 707, with s191 improvement notices accounting for nearly two-thirds (447) of all notices issued.
