Injury highlights risks with line-of-fire positioning and using improvised electrical equipment

The Resources Regulator has issued a safety alert following a serious hand injury incident at a mine site, emphasising the dangers of line-of-fire exposure and the use of improvised electrical equipment.

A worker suffered partial amputation of 3 fingers while performing maintenance beneath a running water cart, to lower the tank after a torque converter seal replacement. The team bypassed these systems, using an improvised drill battery setup to energise a hydraulic solenoid. The tank retracted unexpectedly, trapping the worker’s hand in the mechanism.

The Regulator has published a video featuring recommendations to prevent further incidents from occurring.
