Report on pilot outsourcing of audits for maintenance of competence with practising certificates 2021-2022
Information release
Three workers were exposed to a large flame in a heading in an underground coal mine. It is believed that methane was released from a borehole in the rib that was exposed and then ignited during the coal cutting process.
Policy/Procedure
This policy describes the framework for carrying out a causal investigation, for a quick and full understanding of the causes of safety incidents, and publication of corresponding lessons to reduce the likelihood of recurrence.
