Quarterly safety report released for Oct to Dec 2025

The Quarterly safety report – Oct to Dec 2025 has been released to better inform industry when reviewing safety management systems and undertaking risk assessments at their sites. The report details incidents and trends relating to hazard management, in addition to sector specific information.

As a high-hazard regulator, we focus on compliance with legislative requirements associated with principal and other high-risk hazards.

In this reporting quarter, there were a total of 501 incident notifications received — a 16% decrease from the previous quarter and a 9% decrease from the equivalent period one year earlier.

There was a single fatal incident involving the unintentional detonation of an explosive device which resulted in 2 deaths.

Assessments conducted decreased by 13% overall this quarter from 1,199 to 1,044 across the coal, metalliferous and extractives sectors.

Safety notices issued decreased this quarter from 707 to 542, with s191 improvement notices accounting for nearly two-thirds (318) of all notices issued.
