Investigation information release – worker seriously injured by falling excavator attachment

The Resources Regulator has published an Investigation information release (IIR) concerning an incident that occurred at a quarry in Albury on 12 December 2025. A worker suffered serious injuries after he was struck and trapped by a hydraulic hammer that unexpectedly detached from an excavator.

Mine operators are reminded of their duty to identify hazards and manage risks to health and safety in accordance with provisions of the Work Health and Safety Act 2011 and Work Health and Safety (Mines and Petroleum Sites) Act 2013 and Regulations.
