The Resources Regulator has issued a safety alert and video following a collision between a haul truck and light vehicle.
The incident occurred at a T-intersection at a surface mine. Neither operator was injured in the incident.
Following a review of the incident, the Regulator found several contributing factors including a lack of radio communications, incorrect vehicle safety equipment and sun glare obstruction.
The incident highlights the need for mine operators to review and update any principal hazard management plans for other vehicle operating areas following onsite changes to traffic flow. No route change should commence until controls are field verified and signed off.
Mine operators are reminded to review the Resource Regulator’s Surface Roads or Other Vehicle Operating Areas (ROVOA) Technical Reference Guide (PDF, 4.77 MB).
